The Chronic Disease Management Plan (CDM Plan) is a Medicare rebate funding program for people who have chronic medical conditions (classified as a condition that has been or is likely to be present for 6 months or longer).Your child may be eligible for a Medicare rebate if accessing Speech Pathology services.
What do you need to do to obtain a CDM Plan
- Make an appointment with your GP and discuss whether or not you meet the requirements for a CDM plan.
- If you meet the requirements, your GP will provide you with the paperwork required in order to claim the rebate. *Please note that we charge a gap fee, so you will have some out of pocket expenses.
- The CDM Plan consists of a maximum rebate of $54.60 per session for five sessions with an eligible allied health provider per calendar year.
- You cannot pay the gap with your private health insurance.
- CDM plans need to be shared across allied health providers (i.e. five sessions equals five in total across Occupational Therapy and Speech Pathology if your child is accessing more than one service at a time).
